Complaints Form

This page has the form for you to complete if you would like to make a formal complaint. You can choose to print the page to complete off-line and send in the post or fill it in on-line and send it directly.

Your complaint will be dealt with in the strictest confidence. The Director will inform the complainant on receipt of the formal complaint and contact all individuals involved within 14 days. If the complaint involves the Director, then it will be passed to an independent, impartial professional who will follow the above process.
